To authorize request access to the account or closure of the account, we require one of the following: A signed DocuCare Request form, a copy of a court-issued guardianship or conservatorship order that explicitly grants you the legal authority to manage and close their accounts or a valid power of attorney document that specifically authorizes you to manage and close accounts on behalf of the user.
