To authorize the closure, we require one of the following: A signed DocuCare Request Form to ensure a request form is properly executed, it’s crucial that both parties involved sign the form, indicating their agreement and consent, a signed and dated letter of authorization from the spouse explicitly stating their consent to close account, or a copy of a court order or legal document that specifically grants you sole authority to close the account.
